How do I make a complaint?
You need to make your complaint in writing. You can make a complaint using our online complaints form or by downloading and completing the form which you can send to us by post or email. It is helpful if you include as much information as possible in your complaint, such as the name of the practitioner, their profession and any other information about what happened.
Can I make an anonymous complaint?
We treat all the information you give us confidentially. However, a council may need to show your complaint to the practitioner and ask them to respond, so they can make a thorough assessment. If you make an anonymous complaint, it can make it more difficult for the council to get enough information to assess your concern properly. We encourage you to talk to us about your concerns and what is involved before you decide whether to make an anonymous complaint.
